SCHEDULING INSTRUCTIONS
To schedule an appointment, call or email kathy@midwestvasectomyreversals.com to confirm our availability. Both a $500 deposit and your health history form are required to hold your appointment. Currently we perform two cases each Thursday.
Download a copy of our health history form (see the link below) and email it back to us as soon as possible.
Your deposit is refundable if your request is made more than three weeks prior to the scheduled surgery. Surgery may be rescheduled at any time - however another deposit is required if any changes are made within three weeks of the originally scheduled date.
On the day of surgery, the balance ($2700) is due before the procedure. You may pay this with cash, money order, cashier's check or credit card (Visa, Mastercard). There is a $200 discount for cash payment of the balance the day of surgery, as we have significant extra costs with credit card transactions.
