Please go to your client portal as soon as possible and fill out the health history form there. This is to check for health issues which might pose a problem for your procedure. Your spot is not considered reserved until we have received the Health History Form and Deposit. If you are booking this appointment for another person, please instruct them to fill the questionnaire out as it's important it is as accurate as possible.
Your deposit is refundable if your request is made more than three weeks prior to the scheduled surgery. Surgery may be rescheduled at any time - however another deposit is required if any changes are made within three weeks of the originally scheduled date.
On the day of surgery, the balance ($3000) is due before the procedure. You may pay this with a cashier's check or credit card (Visa, MasterCard, Discover).
We will contact you two weeks before your appointment date with pre- and post-operative instructions. These are also available on our website.
